Ticket to Play Instructions
All athletes are required by the FHSAA to watch four videos and download the four completion certificates. The required videos are listed below.
a. watch the Concussion in Sport video and print Certificate of Completion
b. watch the Concussion for Students video and print Certificate of Completion
c. watch the Heat Illness Prevention video and print Certificate of Completion
d. watch the Sudden Cardiac Arrest video and print Certificate of Completion

PARTICIPATED IN ATHLETICS IN 2019-2020 --> Log in and click, start clearances here!
c. complete or update student info in Step 1
d. upload the FHSAA Physical 2020-2021 EL2 FHSAA Physical in Step 2, Medical History (all pages of the EL2 should be in one upload)
e. upload the certificate of completion for Concussion in Sport in Step 2, Medical History
f. in the"Additional Form Upload Area" of Step 2, Medical History, upload the Heat Illness Prevention, Concussion for Students & Sudden Cardiac Arrest. This must be done as a single upload for the three remaining certificates and not three different uploads.
g. Please keep in mind, you need to allow three to four business days to receive clearance to participate.
Please email any documents (listed below) you are unable to upload onto Athletic Clearance to Buffy McTureous at email@example.com
a. NEW STUDENTS AND 6TH GRADERS ONLY-->A copy of your birth certificate
b. copies of all four aforementioned Certificates of Completion, Proof of Insurance and EL2 form if not uploaded on Athletic Clearance site.
Once you submit your information on athleticclearance.com, you will receive an email stating you have begun the clearance process. This email is not granting your child eligibility. You will receive an email three to four business days following submission of items listed in #3 above for "Ticket to Play Clearance" email. This will mean that eligibility has been granted. Students may not begin open facility, conditioning, tryouts, practices, or athletic competitions without a completed Ticket to Play. Please do not wait until the last minute to complete all the necessary information and steps. It takes three to four business days to receive clearance, after you have completed all the necessary steps.
Turn your Ticket to Play Clearance Email (the second email you receive from firstname.lastname@example.org) into your coach. Your coach will not take any other Ticket to Play paperwork from you. This is your admission into open facility, conditioning, tryouts, practices, or athletic competitions.
Assuming student continues to meet academic eligibility requirements as outlined in the Student-Parent Athletic Handbook, the Ticket to Play Process must only be done one time annually. Following the original submission of a Ticket to Play, students will be placed on a "Cleared List" and permitted to participate in other athletic related activities in the school year.
The FHSAA Physical (EL 2 Form) is ONLY valid for 365 days from the date the exam was done. If it expires before the end of the school year, the student will temporarily lose eligibility until a new form can be provided.
